隨著人口老齡化程度的加深，老年人的健康需求日益凸顯。許多高齡、獨(dú)居、行動(dòng)不便的老年人，面臨著就醫(yī)難、體檢難的問(wèn)題。為破解這一難題，賢文社區(qū)衛(wèi)生服務(wù)中心聚焦老年人“急難愁盼”，將醫(yī)療服務(wù)從“醫(yī)院內(nèi)”延伸到“家門(mén)口”，把醫(yī)養(yǎng)結(jié)合的溫暖送到老年人的心坎上。

　　With the deepening of population aging, the health needs of the elderly have become increasingly prominent. Many elderly individuals who are advanced in age, live alone, or have mobility difficulties face challenges in accessing medical care and health check-ups. To address this issue, Xianwen Community Health Service Center focuses on the "urgent, difficult, and pressing" needs of the elderly, extending medical services from "inside hospitals" to "at their doorstep," delivering the warmth of integrated medical and elderly care to the hearts of the elderly.

　　在入戶(hù)服務(wù)的過(guò)程中，社區(qū)醫(yī)護(hù)人員還化身健康知識(shí)的傳播者。他們用通俗易懂的語(yǔ)言，向老人們講解高血壓、糖尿病的日常護(hù)理知識(shí)，指導(dǎo)老人合理飲食、規(guī)律作息。遇到老人提出的健康疑問(wèn)，醫(yī)護(hù)人員總是耐心解答，消除他們的顧慮。對(duì)于血壓、血糖數(shù)值異常的老人，醫(yī)護(hù)人員會(huì)及時(shí)記錄，并為其制定個(gè)性化的健康建議，必要時(shí)協(xié)助轉(zhuǎn)診，形成從居家監(jiān)測(cè)到專(zhuān)業(yè)診療的閉環(huán)服務(wù)。這種“醫(yī)療+養(yǎng)老”的模式，既滿足了老年人的基本醫(yī)療需求，又給予了他們精神上的慰藉，讓老人們?cè)诩抑芯湍芟硎艿奖憬荨?zhuān)業(yè)的健康服務(wù)。

　　During home visits, community healthcare workers also serve as disseminators of health knowledge. Using simple and easy-to-understand language, they explain daily care tips for hypertension and diabetes to the elderly, guiding them on proper diet and regular sleep schedules. When addressing the health concerns raised by seniors, the medical staff always provide patient answers to alleviate their worries. For those with abnormal blood pressure or blood sugar levels, the staff promptly records the data, offers personalized health advice, and assists with referrals when necessary, forming a closed-loop service from home monitoring to professional treatment. This "medical care + elderly care" model not only meets the basic healthcare needs of seniors but also provides them with emotional comfort, allowing them to enjoy convenient and professional health services right in their homes.

　　The integration of medical care and elderly care services is not only an organic fusion of medical resources and elderly care services but also a robust safeguard for the health rights of the elderly. By offering home-based services such as blood pressure and blood sugar monitoring, it bridges the "last mile" in serving the elderly, enabling them to receive high-quality medical care in their familiar home environments and truly achieving "medical care for the elderly, elderly care for the elderly, and peaceful aging for the elderly."  Moving forward, the Xianwen Community Health Service Center will further deepen the integrated medical and elderly care service model, expand service offerings, and enhance service quality, providing more comprehensive, convenient, and efficient medical and health services to community seniors.
