慢病是威脅人類健康的公共衛生問題，包括高血壓、糖尿病、腦卒中、冠心病壓等。慢病管理防大于治，常規慢病管理主要靠患者自我檢測。但在鄉村，村民本身不夠重視，存在操作不規范、設備老化、遺忘測量、測量時間不規律、缺少長期記錄等諸多問題，導致很難有效實現慢病管理。而到醫院檢查又需掛號，排隊，費時費力，成本很高。所以不少慢病患者漸漸只能是“慢病不管”了。

　　為規范慢病隨訪，在市衛生健康局指導下，市二院醫療集團引進“公共衛生慢病隨訪包”。公共衛生慢病隨訪包的工程師來到醫院，現場為我院公衛人員和轄區內鄉醫進行培訓，現場指導如何使用及操作步驟。培訓后，我院公衛人員積極學習，認真思考，并且下村實際操作，學會操作后幫助鄉醫解決具體操作問題。對于一些鄉醫操作還有些不熟悉的情況，我院積極安排公衛人員，下村為鄉醫講解疑惑并在現場為其實際操作。
